ABOUT
Golden Gateway delivers community-based mobile outreach to older adults 60+ years with undiagnosed depression and other mental health problems. Clients may be physically, linguistically or culturally isolated, home bound or shut-in. Golden Gateway will serve consumers, including the Asian, African American, Latino and American Indian populations.
SERVICES
- In-home assessment
- Case Management
- Treatment including medication management and counseling
- Education for providers working with seniors with Behavioral health problems
- Support to older adults and training to their family and community caregivers
ELIGIBILITY
Available to those 60+ years old and residents of Santa Clara County.
Please contact jbutler@catholiccharitiesscc.org. All Catholic Charities volunteers are required to complete a background check and TB testing.
