Older Adult Outpatient Services Program

Golden Gateway Program

The Older Adult Outpatient Program (Previously known as Golden Gateway) delivers community-based mobile outreach to older adults with serious mental health condition(s) and may have co-occurring disorders. The Older Adult Outpatient Program is designed to help those who are suffering from mental illness, increase functionality, reduce homelessness, increase self-help and client/family involvement. Services include, but are not limited to, individual therapy, individual rehabilitation and case management.

Eligibility

Available 60+ and older, residents of Santa Clara County with full scope Medi-Cal Benefits.

Services Offered

  • In-home assessment

  • Case Management

  • Crisis Intervention

  • Treatment including medication management and counseling

  • Education for providers working with adults and older adults with behavioral health problems

  • Support to adults and older adults and training to their family and community caregivers

  • Medication Evaluation and Management
