CORALAfter-School Instructor
General Function:
Under the direct supervision of a Site Manager, the CORALAfter-School
Instructor (CASI) will be responsible for the supervision and safety of
approximately 20 school age children.Within a small group setting the After School Instructor will help
develop and carry out lesson plans, and work to raise academic achievement within
a balanced literacy model.

GENERAL NATURE OF POSITION: As a member of the Behavioral Health Services team, provides reception and intake for the primary care/behavioral health project at John XXIII Neighborhood Center, explains eligibility for program, orients potential and active clients to program requirements, renders daily operations assistance, collects data and produces reports, provides assistance to the project staff and collaborating staff located in joint sites, communicates and coordinates with collaborators, monitors office expenditures, and completes various clerical duties. GENERAL NATURE OF POSITION: Provides outreach, engagement, support, role modeling and/or aftercare to dually diagnosed participants. Provides chronic care management follow-up with participants. Participates actively in the continuous recovery monitoring of program participants. Conducts group self-help groups. Participates in the clinic service delivery team meetings and planning.

GENERAL NATURE OF POSITION: Provides primary care services to clients and oversees the activity and performance of the medical residents. Collaborates with the Catholic Charities consulting psychiatrist, SJSU nursing faculty, Catholic Charities Clinical Director, the Program Manager for the project and the Director of the Partnership Medical Residency program to implement project program and to achieve primary care service goals. Provides consultation to and training of the project clinical team composed of residents, nurse interns, behavioral health interns and the clinical operations staff members.
