Activity Assistant (Peer Mentor)
Company: Catholic Charities of Santa Clara County Location: San Jose, CA Status: Part time (19 hours) Salary: Close Date: Job Description: GENERAL NATURE OF POSITION: Provides outreach, engagement, support and/or aftercare to mental health, substance abuse and/or dual diagnosis, homeless veterans and their families to engage in treatment, prevent dropping out of treatment and. reducing risk of relapse in the management of their disorders and support. ESSENTIAL JOB FUNCTIONS: • Teams with referral organizations and the Project case manager/therapist to support the client families and individuals to maintain their housing, acquire community services and build relationship with other neighbors in community housing. • Assists case managers to locate clients in order to clients to link them with Veteran’s Administration and community services for veterans in addition to the treatment services of the project. • Engages clients to consider individual treatment and psycho-education services from the Project therapist or other therapists at Catholic Charities or other system mental health and alcohol and drug services as appropriate to the individual’s living situation and stage of change. • Provides field peer counseling to homeless clients in confidential situations • Accompanies clients to needed services • Coaches clients on appropriate behavior in housing, employment, healthcare, benefit and criminal justice situations • Advocates access for clients for employment, housing, healthcare, benefits and other community services • Collaborates with other field service teams for outreach to homeless veteran clients • Participates as a Project team member and with other Catholic Charities Teams - Consults and trains other staff as required on the challenges of homelessness • Records services, maintains client files, completes monthly activity reports, and enters data into AWARDS. QUALIFICATIONS: Required education and experience: • Experience with homelessness, mental health and/or substance abuse disorders as an individual or as a family member • Veteran or family member of veteran • Mental health and/or substance abuse disorders managed for at least one year • Criminal justice experience verified and evaluated for hiring for the position • Bi-lingual skills preferred, e.g. Spanish, Vietnamese, American Sign Language, etc. • Good oral and written English skills • Excellent culturally competent, inter-generational, and interpersonal skills. • Computer skills using Windows Other Requirements: • Driver’s license and insurance for use of CCSCC van when available OR • Ability to use the public transportation system TO APPLY: Please submit resume to jobs@catholiccharitiesscc.org or via fax to 408-944-0276 attn: HR This position description intends to describe the general nature and level of work being performed by people assigned to this job.
