Older Adult Services

Outcomes

Caregiver Services
The caregivers of 95% of older adults suffering from Alzheimer’s disease or other dementia who attended our Day Break adult day care program were able to continue caring for their loved ones at home and avoided placing them in long term care facilities.

Long Term Care Support Services
The Long Term Care Ombudsman Program conducted 1,829 complaint investigations made by or on behalf of residents in long-term care facilities with 98% of those cases being closed, with an average time of 23 days to resolve a complaint.

Senior Community Services
As a result of receiving a small, non-taxable stipend, 74 Foster Grandparents (low-income older adults who work 20hrs/wk with children with special needs) increased their income by an average of 37% for the year.


Economic Development Services

Financial Education
The Tax EZ Program filed tax claims for 1,353 tax payers and helped 80 clients file their back taxes and an additional 25 tax payers apply for ITIN (Individual Tax Identification Number.) Tax payers received a total of $1,503,359 in refunds and $685,612 in EITC. Each household received an average of $1,111 refunds.

Refugee Resettlement
84 refugees were successfully resettled and helped with housing, education, employment, and social services.

Refugee Foster Care
Through our Refugee Foster Care program, 25 refugee minors were able to leave the confines and conditions of refugee camps and be placed into loving foster homes. All now have access to higher education. One is just starting at SCU on a scholarship

Immigration Legal Services
Through our Immigration legal services over 3,000 clients received low cost quality immigration legal services and gained knowledge of their legal rights and options

Housing Services
Funding was secured for housing for 29 chronically homeless individuals with disabilities and additional funding for 14 chronically homeless families with disabilities ensuring greater safety and stability.


Youth and Family Services

El Toro Youth Center
Ninety-one percent of children in the Center’s programs improved their grades in school.

Intervention Services
A new grant from the county probation department helped 209 youth on probation stay out of jail. The program doubled the number of youth served, achieving 200% of its goal.

Washington United Youth Center
Ninety percent of WUYC youth reported staying safe and getting along better with their families.


Educational Services

CORAL After-School
More than 518,0000 literacy hours were delivered to the 3,162 students attending the CORAL programs at 20 school sites … contributing to a one year average reading gain for each child.

Family Support
More than 81% of 3,000 parents reported an increase in their knowledge about the importance of literacy; the benefits of reading with their children; and the use of public libraries. Thanks to the First 5 parenting program, parenting skills increased for 82% of the parents involved. The Family Partners Program well exceeded our goal of 75 families by serving a total of 215 families.  We were able to triple the amount of children screened for developmental and social emotional needs.


Behavioral Health Services

Supportive Housing Services
One hundred percent of tenants retained their housing.

Adult Substance Abuse Program
Ninety-one percent of clients have reduced their alcohol or other drug use.

Navigator Homeless Services
The program has helped 120 homeless individuals with mental illness and/or drug abuse issues access services.

Adult Mental Health Services
On hundred percent of CalWorks clients in treatment have not experienced relapses.
Eighty-eight percent of adults in the program have avoided psychiatric hospitalization and maintained stable housing.
Ninety-one percent of older adults in the program have avoided psychiatric hospitalization.

Focus for Work
The program exceeded contract goals with 64 closures and 132 employment placements of adults with severe mental illness.
