Description and Services:

We provide Case Management for tenants living in specific Charities Housing Development Corporation apartments. Supportive Housing Case Management Services assists tenants in maintaining their housing.  Services include:

  • Counseling clients in their homes
  • Helping clients access services they need
  • Assessing client needs and resources, creating action plans, and evaluating outcomes
  • Organizing community-building activities for residents of apartment buildings

Eligibility and Requirements:  Low-income families and individuals or single parents with children

Waiting List: There is a waiting list according to the availability of the property

Fees: None

Hours and Availability: Monday – Friday, 9 a.m. - 5:00 p.m.
