Older Adult Services

Summary of services:

• Evaluation and assessment of an individual’s mental health needs and strengths, and the interaction with physical health needs

• Case management, including linking individuals to necessary medical, community, and social services

• Medication evaluation and support

• Individual counseling

• Communication with family members as appropriate

Who is eligible?

• 55 years of age and older

• Santa Clara County residents

• Elders experiencing emotional problems and/or mental illness that affect the ability to perform daily activities.

Is there a waiting list?

There is no waiting list at this time.

Are there any fees?

Medi-Cal is accepted, or the County’s sliding fee scale is applied according to the individual’s ability to pay.

What volunteer opportunities are available?

Due to the confidential nature of the program, volunteers are not utilized in the program at this time. Clients are linked to community resources for volunteerism.

Q: Where do I go to receive Oasis’ services?

A: Since the Oasis staff understands that it is often difficult for older adults to get to appointments, most program services are provided in the client’s home. The only services that are not offered in the home are counseling sessions and medication support services.

The office is located at 2625 Zanker Road in San Jose. Office hours are Monday through Thursday 8:30 p.m. to 5:00 p.m.
