Description and Services:
We provide Case Management for tenants living in specific Charities Housing Development Corporation and Mid-Peninsula Housing Corporation apartments. Supportive Housing Case Management Services assists tenants in maintaining their housing. Services include:
- Counseling clients in their homes
- Helping clients access services they need
- Assessing client needs and resources, creating action plans, and evaluating outcomes
- Organizing community-building activities for residents of apartment buildings
Eligibility and Requirements: Low-income families and individuals or single parents with children
Waiting List: There is a waiting list according to the availability of the property
